  • Booking Fee: $50 (Non-refundable. Credited to Certification Visit fee if appointment kept.)
  • Initial Certification Visit: $250 for 7 months or $200 if paid in full at booking, $150 if US veteran/cancer diagnosis.
  • Re-certification/Transfer Visit: $150 (every 7 months), $125 if US veteran/cancer diagnosis.
